Infirmary, UK A 62-year-old man was referred with evidence of sepsis secondary to an ulcer over the right lateral malleolus Duplex scanning revealed a long occlusion of the right superficial femoral artery, and the patient underwent a right femoropopliteal bypass (venous-conduit) Despite negative cultures and broad-spectrum antibiotics his pyrexia persisted and the ulcer failed to improve. Subsequent biopsy returned a diagnosis of an Alk-1-negative anaplastic large cell cutaneous lymphoma. Chemotherapy was commenced to good effect. One should consider the possibility of an alternative diagnosis early when revascularisation does not improve the clinical picture in patients with critical limb ischaemia
